Intent: The purpose of this assignment is for students to apply what they learnt in this subject about primary health care and social determinants of health to investigate and report on a primary health service in their community.
Task: To complete this assessment, you are to identify a Primary Health Care service in your community. This primary health care service should employ nursing staff in some capacity. Alternatively, you can discuss how nurses could be integrated into the primary health care service to improve health outcomes. You are to describe the following aspects of the chosen service
- Service profile and access – Who uses the service? What service is provided to the community? How is this service funded? Do clients pay to access the services?
- Health impact – How does this service impact on the health of the clients they engage with? Reflect on the role of the health practitioner in this service and describe the impact they can have on the health and wellbeing of clients accessing this service.
- Social Determinants of Health (SDH) – Which SDH are influenced by this service? How can you measure the impact this service has on these SDH?
- Nursing scope – What role do/could nurses have in this service? How do/could they contribute to better health outcomes for people who use the service? Are there any ways this scope could be increased or optimised?
